WiSE from the 1st January 2011 will have a Service Level Agreement(SLA)
with The British Red Cross Society to fund the Home from Hospital service.
This is our very first SLA agreement with the Red Cross. The service offers
support to people on discharge from hospital. If you are able to volunteer
to help this service develop then please contact:

Care in the Home
Service Description
To provide a bespoke support for people over 60 years who are unable to
manage at home alone after a spell in hospital or day hospital treatment.
This service will also contribute to minimising the risk of re-admission.
Support visits will be made once or twice a week, depending on need for
a maximum of 6 weeks, to help regain confidence and independence.
Working from the offices of Wetherby in Support of the Elderly and Harrogate
and Leeds Hospitals. Referrals will be taken from both community and hospital
health care settings.
Service Specification
Home Support:
Check Discharge instructions are understood.
Within 48 hours of discharge (72 hours at a week end or Bank Holiday) a
member of the Home from Hospital team will make contact with the individual
either by telephone or home visit.
To visit or telephone on an agreed frequent basis, reducing contact over
the 6 weeks.
Shopping:
To collect shopping prior discharge
To collect shopping for the individual during the period of disability /
confinement.
To accompany an individual to the shops to help them regain confidence /
aid them.
Prescription Collection:
To collect a prescription for the individual during the period of disability
/ confinement.
To accompany an individual to the chemist to help them regain confidence
/ aid them.
Cleaning:
To undertake light cleaning duties e.g. hoovering, light dusting, washing
up, emptying the washing machine.
To work with the individual, undertaking light household tasks.
Assist with Paperwork:
To help with any form filling and signposting to other agencies.
Information and advice:
To provide appropriate information and advice when required.
To assist individual to find out necessary information.
Volunteer Training:
All volunteers will undertake an enhanced CBR check and will attend appropriate training.
